Commander, MTB Squadron 34, submitted the CONFIDENTIAL report to Commander in Chief, United States Fleet, via Commander, PT Squadrons, Eleventh Amphibious Force, and Commander, Eleventh Amphibious Force. File FC8-34/A12-1; reference COMINCH letter FF1/A12-1/A16-3, serial 7152 of 29 October 1943. The final page bears H. J. Sherertz’s name and a copy notation for CTF 125. No separate endorsements are included in this four page set. [D pages 1 and 4]
